Easiest Steps on How to Apply and Register For a PhilHealth ID

Do you wish to be a member of PhilHealth and want you and your beneficiaries avail of the benefits?

PhilHealth is the leading health insurance company in the Philippines. It aims to support them by providing medical and hospitalization assistance whenever they need to avail of it.

We’ve gathered the steps and requirements you need to become a member of PhilHealth:

1. Requirements for Unemployed Members and Employed Members

If you are unemployed or self-employed, you can apply as an Individually Paying Member.




Submit the ff:

– Accomplished and properly filled out (2) copies of MRF (Philhealth Member Registration Form)

-Two (2) 1×1 Latest ID picture

– Photocopy of NSO Birth Certificate or Any Two Valid IDs

-Photocopy of supporting documents for your beneficiaries

Submit the requirements and you will be given Philhealth number and will be asked to pay your contributions

2. Requirements for Employed Members

The requirements for employed members are the same as listed above for Individually Paying Members. Just submit and include your employed status in the PMR.

3. Requirements for OFW

For OFW (Overseas Filipino Workers), you can register and pay your contributions one you register at the POEA as OFW.



Submit the ff:

-Accomplished and properly filled out (2) copies of MRF (Philhealth Member Registration Form)

-Two (2) 1×1 Latest ID picture

– Photocopy of NSO Birth Certificate or Any Two Valid IDs

-Supporting documents of beneficiaries

-Any proof of being an active OFW

You will receive your Philhealth number afterward, which you should use when paying for monthly contributions.



PhilHealth ID CARD

Aside from the PhilHealth ID Card, existing and aspiring members can also apply for the new PhilHealth card loaded with perks.

Members can also process dependent ID Cards at any SM Mall for 360 pesos. Members can claim this ID for 90 at any Philhealth LHIO. If you wish to declare a dependent for your ID Card, you must head to any Bayad Center outlet and pay 358 pesos and present at least two (2) valid IDs of your dependent.

Apart from hospital discounts, members with a PhilHealth Insurance ID Card can avail of the following perks:

-15 % Discount on generic drgs on Watson’s, Rose Pharmacy, South Star and The Generics Pharmacy

-20% to 80% discount on flu vaccines on PQ Health Shield and GSK

-Free eye exam from Vivian Sarabia Optical

-10 % discount on testing services at JNW Drg Testing Sites

Also note that this Philhealth ID is a valid government ID, which you can use for bank account applications and other organizations that require proof of identity.
